STUDENT-ATHLETE INFORMATION: 

2024-2025 Pre-Participation Physicals: All athletes 7^th- 12^th grade will need an annual physical on file prior to any athletic participation.  For PPE’s to be current for the 2024-2025 school year, they will need to be done AFTER May 1^st, 2024 on the updated 2024 UIL PPE Form.

Rank One Student Online Medical Forms: All parents/guardians of athlete’s 7^th-12^th grade will need to complete the online medical forms prior to any athletic participation.  Rank One forms for the 2024-2025 School Year will be ready to complete AFTER MAY 20^TH, 2024.  Any Rank One forms uploaded and/or completed prior to May 20^th will not transfer over to the new school year and will need to be redone. 

Uploading Physicals to Rank One:  AFTER May 20^th, please upload your completed physical form (both front and back pages) into Rank One while completing your other forms.  By uploading an electronic copy, you will not need to submit a hard copy to your coach or the Athletic Trainers.  Please double check that all sections have been completed and both Student and Parent/Guardian signatures are on the bottom of the front Medical History page.  

SPORTS MEDICINE INFORMATION: 

   Mondays - Thursdays: 7:30am-8:00am in the HS Training Room

   All other treatments will be during your athletic period or by appointment only

Doctor’s Clinic: (available for all Anna ISD Athletes 7^th-12^th grade injured in their school sport)

Wednesdays: 7:30am-8:00am in the HS Training Room by appointment only

Physical Therapy Clinic: (available for all Anna ISD Athletes 7^th-12^th grade injured in their school sport)

Thursdays: 7:30-8:00am in the HS Training Room
